Sebastian Keith Parman died in Geraldton Regional Hospital on September 17, 2010, soon after he was diagnosed with scarlet fever, even though the cause of death was later on recognized by a coroner as issues linked with swine flu.

Ms Piani, also a registered nurse, brought Sebastian back again to the unexpected emergency section exactly where he introduced with a fever, large coronary heart charge and a rash.
The inquest read the doctor examining Sebastian handed over the case to Dr Cupitt prior to ending his shift.
“Accounts vary, but the result of the discussion seems to have left Dr Cupitt with the perception that she was to assessment Sebastian’s upper body X-ray and to permit him go property once she had checked it,” Ms Ellson mentioned.
“Later in the day, Ms Piani questioned Dr Cupitt if she could just take Sebastian house. Without inspecting him, Dr Cupitt checked the chest X-ray and Sebastian was permitted to go home.”
Ms Piani sought a second impression for Sebastian the following day after his condition appeared to worsen.
